Complete the Heritage Health Index Questionnaire Online

Institutions were sent a unique password in their survey package for accessing the Heritage Health Index online at www.heritagehealthindex.org.

Replying online has helpful features for the user, including the ability to save responses so that the questionnaire may be completed in multiple sittings, indicators of which sections are completed and which ones are in progress, and one-click access to definitions and Frequently Asked Questions. The online version provides an easy way to have several staff members contribute to data collection. In addition, there are tools such as a calculator that computes full-time equivalent (FTE) staff.

To encourage Heritage Health Index participants to submit their survey online, they will be given access to a running tally of returns by state and type of institution and some of the preliminary results. Institutions will be able to print their completed questionnaire for their records, and data submission will be instant and confidential.
